What the inspector wrote

An Annual visit was conducted by Licensing Program Analyst (LPA) K. McClurg. LPA met with Administrator (Admin) Caroline Alacar & Program Manager (PM) Jason Gavilan, greeted both, stated purpose of visit, & was allowed entry. This facility has a wood burning fireplace that is not used & does not have a gas starter or plumbed for gas. This facility has no open bodies of water such as pools, spa, fountains, etc. This facility is fire cleared for all Non-Ambulatory residents. Physical plant toured. Kitchen appeared to be clean & organized. Sufficient supply of perishable & nonperishable food on the premises. Refrigerator & freezer appeared to be clean & maintained at appropriate temperatures. Food appeared to be stored in a safe and healthful manner. Knives & cleaning products stored in separate locked areas making inaccessible to residents. Front room & Dining/living room sufficiently furnished with adequate lighting. Furnishings appeared to be in good repair. Resident bedrooms sufficiently furnished with adequate lighting. Resident bathrooms appeared to be clean with all fixtures in working order. No unpleasant odors. Grab bars in toilet & tub/shower areas. Sufficient supply of linens. Continued. (Continued) Interior passageways & exits observed to be clear with no obstructions Interior passageways & exits observed to be clear with no obstructions, including exit door in each resident bedroom. Smoke & carbon dioxide detectors operational. Fire extinguisher service date: 6/26/25. Outside area clear of debris. Front & backyard sufficiently maintained. Sufficient seating in good repair. Medications observed to be inaccessible to clients & organized. Medication Administration Records (MARs) & Centrally Stored Medication & Destruction Records (CSMDRs) documentation completed & maintained. Resident files maintained. Staff files maintained. Admin (Caroline Alacar) Residential Care Facility for the Elderly (RCFE) Administrator Certificate #7013451740 expires: 4/18/27 & current 1st-Aid/CPR expires: Jan 2027 PM (Jason Gavilan) Administrator Certificate #7023285740 expires: 11/11/26 & current 1st-Aid/CPR expires: 4/4/27. Exit interview conduced with Admin, & PM. Report provided.
