What the inspector wrote

On 12/15/2025 Licensing Program Analyst (LPA) B. Miranda arrived to the facility unannounced to conduct an annual inspection. LPA met and spoke with House Lead Felicha Sanchez. Licensee Paulette Holland was contacted and arrived shortly after. Upon arrival LPA observed residents eating breakfast in the dining area at the kitchen table. Facility is licensed for 6 residents and have a current census of 5. The facility has 3 bedrooms and 3 bathrooms. LPA observed the facility to be clean and odor free. LPA tested the water temperature in the kitchen which read at 114.2. LPA observed a fire extinguisher in the kitchen which was last serviced 12/6/2024 and has current charge. Carbon monoxide detectors and smoke detectors were tested and are in working order. LPA reviewed disaster plan which is completed but has not been marked as recently being reviewed or updated. LPA did not observe an infection control plan at the facility and Licensee Paulette Holland confirmed there was not currently an infection plan at the facility. LPA reviewed all resident files. R1 is missing a care plan for their oxygen and there is no verification of staff training, residents do not have a physician's report on file within the past 12 months. LPA reviewed a sample of employee files, S1's file did not have current training in file. LPA observed a TV, multiple walkers, and wheelchairs in the outside patio area. LPA observed scissors and screw drivers in unlocked drawers in the kitchen. Due to time constraints LPA did not cite and will return to cite at a later date. Exit interview was conducted via phone and a copy of this report LIC809 was provided to Licensee Paulette Holland.
