What the inspector wrote

Licensing Program Analyst (LPA) Katie Brown arrived unannounced at the facility to conduct a Case Management visit. LPA met with and explained the purpose of the visit with Health Services Director (HSD) Anjeanette Franco. Administrator Heidi Setty arrived shortly after. The purpose of the Case Management visit is to follow up on a Special Incident Report (SIR) submitted to CCLD. The incident occurred on 10/7/21 involving R1. LPA interviewed staff and conducted a record review of facility documentation related to the incident. No deficiency cited See Technical Advisory A copy of this report was provided and an exit interview was conducted with Administrator.

Incident dates include 11/12/21, 11/13/21, 11/15/21, 11/28/21 and 9/14/21.This poses a potential health & safety risk to persons in care.
