What the inspector wrote

Licensing Program Analyst (LPA) Katie Brown arrived at the facility unannounced to conduct a Case Management - Incident follow up. LPA met with and explained the reason for the visit with Memory Care Director (MCD) Andrea Yascas. On 9/4/23, the facility submitted a SOC341 reporting an altercation between Residents R1 and R2. During the visit, LPA reviewed both resident’s files. Resident Assessments are in process of being updated and required notifications have been made. There were no citations issued An exit interview was conducted and a copy of this report was left with MCD, whose signature confirms receipt of these documents.

Cleaning supplies and laundry detergent were observed in 116.This poses an immediate health, safety or personal rights risk to residents in care.

    87465 Incidental Medical and Dental Care (h) The following requirements shall apply to medications which are centrally stored: (2) Centrally stored medicines shall be kept in a safe and locked place that is not accessible...The requirement was not met as evidenced by: Licensee did not ensure that medications were centrally stored and locked. Over the counter medications were observed accessible to R1 and R2. Required documentation and assessments were not maintained in resident files.This poses an immediate health, safety or personal rights risk to persons in care.
