What the inspector wrote

Licensing Program Analyst (LPA) Katie Brown arrived unannounced to deliver investigation findings. LPA met with and discussed the allegation with Administrator (AD) Heidi Setty. This Department investigated the allegations listed above. Interview with the Administrator (AD) confirmed that any resident can receive any type of therapy once ordered by their Physician.R1's Physician has not ordered therapy and R1's Responsible Party has not requested the facility initiate this service for R1. A review of R1's current Physician ordered medications was conducted. An audit was conducted to confirm that medications ordered and available for use. The facility maintains communication with R1's Physician and Responsible Party as required. This Agency has investigated the allegations listed above. We have found that the allegations are UNFOUNDED, therefore we have dismissed the allegations. There were no citations issued. An exit interview was conducted and a copy of this report was provided.

(6) to leave or depart the facility at any time… This does not prohibit a licensee from establishing house rules..., This requirement was not met as evidenced by: Licensee did not ensure all residents could leave the facility at any time. On 6/29/25, R1 was restricted from leaving the facility to attend church per a restriction placed without a court order by the Public Guardian. This poses a potential health & safety risk to persons in care.
