Licensing Program Analyst (LPA) Katie Brown arrived unannounced at the facility to deliver investigation findings. LPA met with and explained the purpose of the visit with Administrator Donna Hurley. The Department investigated the allegation: Facility was unable to conduct a timely assessment in order for resident to be discharged from hospital. The Department conducted multiple interviews resulting in conflicting information. Based on interviews and record review of the hospital Clinical Notes, the hospital was in communication with the facility administration and R1’s Responsible Party multiple times to plan the discharge of R1. Based on interview and record review, the allegation is UNSUBSTANTIATED. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove that the alleged violation did or did not occur. A copy of this report was provided and an exit interview was conducted with Donna Hurley.
