Inspector’s narrative

What the inspector wrote

On 01/17/2023, Licensing Program Analyst (LPA) Walton arrived unannounced to conduct an Annual Inspection- Infection Control. LPA introduced self, stated the purpose of the visit and requested to meet with the Administrator. Facility staff contacted Administrator, Rosemarie Reimer via telephone. Administrator and Licensee, Balwinder Kaur, arrived a short time later. Facility has one central entrance and exit. Facility tour conducted. LPA toured the facility kitchen. LPA observed a chain and padlock around the refrigerator and non-perishable food to be locked in the staff room. LPA observed cleaning supplies to be accessible in a cabinet under the kitchen sink. Food supply checked. LPA observed an adequate supply of PPE and cleaning supplies. Medications checked. Medication observed to be locked and inaccessible to residents in care. LPA did not observe a trash can with lid in bathroom 1 and hand washing signs were not observed in bathroom 1 and bathroom 2. Bathrooms were stocked with liquid soap and paper towels. Fire extinguisher was last serviced on 08/25/2020. Resident bedrooms were checked. LPA observed the following in bedroom two: light switch to be in disrepair, the ceiling fan light fixture cover was missing and the light chain was wrapped around and stuck inside the ceiling fan, preventing the light to be turned on. LPA observed the smoke detector to be missing with an object hanging from the electrical wire. Facility tour continued. LPA observed a bed and a night stand to be blocking the exit in room three. The screen on the exit sliding glass door of room three was in disrepair. LPA observed cleaning supplies and a razor accessible to residents in care in the bathroom of bedroom 4. Continued to 809C LPA is requesting the following documents be submitted to the Fresno CCL office by 01/31/2023: Current copy of Administrator Certificate, Designation of Facility Responsibility (LIC308), Administrator Organization (LIC 309), Affidavit regarding Client/Resident Cash Resources (LIC 400), Liability Insurance, Emergency and Disaster Plan (LIC 610E) Personnel Report (LIC500), Register of Facility Clients/Residents for (LIC9020A), Surety Bond. Deficiencies are being issued in accordance to California Code of Regulations, Title 22, Division 6 on the attached 809D. A civil penalty in the amount of $500 is being assessed for fire clearance. Exit interview conducted and a Plan of Correction was reviewed and developed with Administrator and Licensee. A copy of this report was discussed and provided to Licensee, Balwinder Kaur, whose signature on this form confirms receipt of this document.

    Based on observation, the licensee did not comply with the section cited above when cleaning supplies were accessible to residents in care in bathroom 2, and under the kitchen sink which poses an immediate health, safety or personal rights risk to persons in care.

    Based on observation, the licensee did not comply with the section cited above when the facility refrigerator was observed with a chain and padlock preventing access to residents in care which poses a potential health, safety or personal rights risk to persons in care.
