What the inspector wrote

Although the allegations may have happened or are valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur. No deficiencies issued. Exit interview conducted. A copy of this report was discussed and provided to Administrator, Minakshi Roychoudhury, whose signature confirms receipt of this document.

  • Authorized physician-prescribed self-administered meds

    (a) (5) Facility staff… may assist persons with self-administration as needed. Assistance with self-administered medications shall be limited to the following: (A) Medications usually prescribed for self-administration which have been authorized by the person's physician… this requirement was not met as evidenced by: Based on observation and record review, the licensee did not comply with section 87465 when over-the-counter medication was observed stored in the facility and administered without a prescription from a physician.

    Dispose of unused medications with required witness

    87465 Incidental Medical and Dental Care (i) Prescription medications which are not taken with the resident upon termination of services, not returned to the issuing pharmacy… shall be destroyed…by the facility administrator and one other adult who is not a resident. Both shall sign a record, to be retained for at least three years… this requirement was not met as evidenced by: Based on observation, the Licensee did not comply with section 87465 when medication belonging to a resident who no longer resides in the facility, was observed co-mingled with current resident medication, which is an immediate health and safety risk to persons in care.
