What the inspector wrote

Licensing Program Analyst (LPA) Sarah Hurt conducted an unannounced facility visit to conduct a Case Management. LPA met with facility staff, Tammy Pirhekayati, and explained the purpose of today's visit. LPA Hurt informed facility staff if they do plan to sell this facility they will need to contact Centralized Applications Bureau, and follow the required process. LPA Hurt informed the facility staff if they plan to change the facility name they will need to submit the LIC200 form to State Licensing. The facility must ensure all advertising's are using the name they are licensed (Helping Hands.). No deficiencies cited during visit. Exit interview conducted with facility staff Tammy Pirhekayati, and a copy of this report provided.
