On 05/19/25, Licensing Program Analyst (LPA) M. Yang arrived unannounced to conduct case management visit for the purpose of checking on the health and safety of the clients in care. LPA introduced self, stated the purpose of the visit and met with Administrator Karen Dhaliwal. LPA was informed Administrator, that excluded individual is not employed with facility and have been taken off schedule since 12/16/24. Individual is not associated with facility. No deficiency cited during visit. A copy of this report was provided to Administrator. Exit Interview.
