Pre-licensing visit·December 12, 2025

On 12/12/2025, Licensing Program Analyst (LPA) M. Garza arrived at the facility for an announced Pre-licensing visit. LPA was greeted by Licensee Ronito Galvez and Administrator, Carlo Santos and was allowed entry in the facility. LPA conducted the inspection using the Inspection Tool. LPA toured the facility inside and out with Licensee and Administrator. The facility is a 6 bedroom and 3 bath home. Fire clearance was granted for a total of 5 non-ambulatory residents and 1 bedridden resident. There are no residents in care at this time. Facility's phone number is (559) 724-9855. Upon entry, LPA observed the required postings to be present. Common areas observed to be well lit and have adequate furnishings. Bedrooms observed to have required furnishings and lighting. Mattresses appear to be in good condition. An extra supply of linens were located in the hallway closet. LPA observed locked closet in the hallway identified as the medication closet. Kitchen observed to have dishes, plates, and utensils. Knives locked and secured in the kitchen drawer. LPA observed a supply of food, emergency food and water. Supply of personal hygiene, grooming products, cleaning supplies and chemicals located in a locked closet in hallway. First aid kit contained all required items. A fire extinguisher was purchased on 05/07/2025 and observed fully charged. Smoke detectors and carbon monoxide detectors present and operational at time of visit. Exits were open and free of obstructions. LPA observed side gate to be unlocked and self-latching. A covered patio area and activity space observed in backyard. During visit the following issues were observed: Patio in back yard and right side yard observed to have a 4-6 inch drop from concrete in need of filling/hand railing. Facility does not have an internet access device for residents. A return visit has been scheduled for 12/16/25 @ 3pm to ensure corrections have been made. No deficiencies cited during todays visit. Exit interview completed with Licensee and Administrator and a copy of this report was provided.
