What the inspector wrote

Based on records reviewed, facility conducted a reappraisal for resident 1, R1, on 05/16/2023. The reappraisal was requested by responsible party after observing a change in condition. Facility updated resident care plan to address additional needs. LPA did not find supporting documentation to show facility did not meet this residents needs. Although the allegations may have happened or are valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur, therefore the allegations are Unsubstantiated.
