Licensing Program Analyst (LPA) Esther Miller conducted an unannounced case management visit. LPA was granted entry into the facility and met with Gabby Zamora, House Manager, with whom she discussed the purpose of the visit. During today's visit, LPA briefly toured the facility and conducted and health and safety check on the clients in care. House Manager self-reported to Community Care Licensing (CCL) on August 11, 2023 that Resident 1 (R1) had taken an absence without leave (AWOL). R1 was able to leave the facility unnoticed because exit did not have a door alarm. Physician's Report dated June 21, 2023 showed that R1 had mild cognitive impairment (MCI) and could leave the facility without assistance. House Manager indicated that, due to cognitive concerns, staff contacted law enforcement right away. R1 was found the same day and returned to the facility. House Manager stated R1 was located in the backyard of an unrelated person and R1 could not explain where they were going or why. House Manager has since updated R1's Needs and Service Plan as well as obtained an updated physician's report. On August 16, 2023, House Manager indicated that the auditory device (aka door alarm) to monitor exits was working, but was constantly going off because the doors could not fully close since April 8, 2023. Due to this, the alarm was turned off. House Manager stated that Licensee was aware the door was not working and was given authorization to fix the door on August 14, 2023. House Manager stated that the door will be fixed August 21, 2023. A technical violation (LIC9058) was issued. An exit interview was conducted with House Manager and a copy of this report, Technical Violation (LIC9102) and Licensee Appeal Rights (LIC9058) were provided.
