On 02/03/2023 LPA verified and observed that corrections were made to these areas. Staff and residents interviewed stated that the elevator did not work properly and needed a switch to be replaced. Residents that needed additional assistance were moved from the second floor to the first floor. Records revealed the elevator has been under negotiations for repair or to be replaced for the past several months; however, this has not yet been addressed and elevator remains inoperable. The continued lack of elevator maintenance services impacts residents and facility operations. Observations, record review and interviews concluded the preponderance of evidence standard has been met, therefore the above allegations are found to be SUBSTANTIATED. An exit interview was conducted with Gabriela Zamora, Manager. A copy of this report and Licensee's Rights (LIC 9058 03/22) were provided to the Administrator and her signature on this report confirms receipt of the Licensee Rights.
