Licensing Program Analyst, David Roman (LPA D. Roman) conducted an unannounced case management visit to obtain additional information on an unusual incident report made on 06/23/2025. LPA D. Roman conducted an interview with Administrator, Gabriela Zamora. LPA D. Roman also reviewed the residents file and toured the facility along with facility staff. No deficiencies were cited during today's visit. An exit interview was conducted with Administrator, Gabriela Zamora, to whom a copy of this report and the Licensee/Appeal Rights were provided during the visit. Their signature acknowledges the receipt of this report and their rights.
