What the inspector wrote

(Continue from LIC9099) The complaint also alleged that the facility's fire doors were obstructed. The investigation did not yield evidence to support this allegation. During interviews, staff indicated that they did not recall observing any obstruction of fire doors or hallways. During the facility visit conducted on January 10, 2026, no obstructions of hallways or any other areas of the facility were observed. The investigation included a review of images provided by an outside source, which showed facility hallways cleared of any obstruction and doors wide open. The images did not indicate any obstructions or violations of Title 22 regulations. Additionally, the date and time of the images were not provided, limiting the ability to corroborate the complaint. The review of the sample of admissions agreement did not disclose any violations. Based on the investigation, including record review, staff interviews, and observation, there was insufficient evidence to substantiate the allegations. Therefore, the allegations are deemed unsubstantiated. An exit interview was conducted with Med Tech, Veronica Valadez. A copy of this report and the Licensee Appeal Rights (LIC 9058 03/22) were mailed to the licensee of record and emailed to sonrisavillainc@gmail.com
