Licensing Program Analyst, David Roman (LPA D. Roman) conducted an unannounced case management visit to follow up on an incident that occurred on 02/14/26. LPA D. Roman gained access to the facility, identified himself, and met with Temp. Facility Manager, Robbie Cantoria to discuss the purpose of today's visit. LPA D. Roman's visit consisted of interviews with the resident that witnessed the incident and with the victim. LPA D. Roman attempted to speak with the perpetrator but they were not present in the facility. LPA D. Roman met with Facility Manager and conducted a tour of the bottom and top floor of the facility. LPA knocked on the perpetrators door in attempts to interview them to no avail. Facility staff reported that the perpetrator was out of the facility. An exit interview was conducted with Wellness Coordinator, Johanna Caldera to which a copy of this report along with Licensee/Appeal Rights was provided to them whose signature below confirms receipt of this report and their rights.
