Licensing Program Analyst (LPA) David Roman conducted a case management visit for the purpose of monitoring the facility closure. LPA met with Temp. Manager, Tim Aldanese to discuss the purpose of the visit and to obtain additional information in regards to the progress of the facility closure. During today's visit LPA along with facility staff conducted a tour of the interior and exterior of the property to confirm the closure of operations. LPA D. Roman confirmed that the Temp. Manager, Tim Aldanese has terminated all operations and collected the site license. LPA also confirmed that there are no residents in care. All medication has been cleared out of medication room. According to the Temp Manager, all residents have been transferred to other facilities and one resident returned to their family. An exit interview was conducted with the Temp. Manager, Tim Aldanese, Licensee Rights along with a copy of this report was provided to Administrator and their signature on this form confirms receipt of the report and their rights.
