Inspector’s narrative

What the inspector wrote

On 05/20/25, Licensing Program Analyst (LPA) M. Yang arrived unannounced to conduct an annual visit. LPA introduce self, stated the purpose of the visit and requested to meet with Administrator. LPA met House Manager (HM)/ designated representative Heidi Snavely. LPA toured facility with designee. Licensee Marguerite Bruton was called and unable to attend meeting. All five residents were present in the living room during inspection. The facility was observed to be at a comfortable temperature, clean, in good repair, and no passageway obstructions or fire hazards were observed inside or outside. All residents were reviewed. Medications were observed locked in kitchen shelves. LPA reviewed MARS and checked medications. An adequate supply of perishable and non-perishable food was observed. Refrigerator temperature is maintained at 32 degrees F and freezer at -2 degrees F. Fire extinguisher was observed with purchase date of 03/29/25. Extra linens were observed. Approximately 11:31AM, Chemicals were observed unlocked in garage. All bedrooms were observed to have the required furnishings and with adequate lightening. The bathrooms were toured and observed operational during inspection. Non-skid mat and grabbed bars were observed. Hot water temperature was tested at 115.3 ang 115.8 degree F in the master bedroom and 114.4 degree F in hall bathroom. Outside of facility toured and observed to be free of debris. Side gate observed self-closing and self-latching. Adequate outdoor seatings available for residents. At approximately 11:38AM, gardening tools were against facility wall outside unlock. Smoke detector and carbon monoxide was tested operational and functional during inspection. A deficiency is being cited, per California Code of Regulations, Title 22, Division 6, see attached Lic 809D. A civil penalty is being assessed see attached Lic 421IM. Exit Interview conducted. The following documents are requested and submitted to Fresno CCL by: 05/23/25. Forms requested: Lic 308, Lic 400, Lic 402, Lic 500, Lic 610E, control of property, current Administrator Certificate, and current liability insurance. A copy of this report and appeal rights was provided to the designee, whose signature on this form confirms receipt of this report.

This report shall include the resident's name, age, sex and date of admission; date and nature of event; attending physician's name, findings, and treatment, if any; and disposition of the case.This requirement was not met as evidenced by: Based on record review and interview, Licensee did not submit a written report to the department within 7 days of incident when R1 had went to the hospital on 01/06/25, 01/24/25, and 04/08/25, this poses a potential health and safety risk to residents in care.

    Based on records reviewed and interview conducted, the Facility has overdue Annual and Late Fees, this is poses/posed potential health and safety risk to residents in care.

    Ensure hazardous items are locked and not unattended

    Based on observation, the Licensee did not comply with the section cited above when LPA and S1 observed chemicals unlock in the garage accessible to residents in care this poses an immediate health, safety or personal rights risk to persons in care.

  • Give PRN medication by physician order

    Based on observation and records reviewed, R1’s medications Lisinopril 20 mg and Clonidine Hcl 0.1 were not administered as directed by physician, which poses/posed an immediate health and safety risk for the person in care
