On March 3, 2022 an office meeting was conducted virtually, via Microsoft Teams. Present during the virtual meeting are: CCLD staff from Sacramento South RO, Fresno RO, CCLD Audit Section and Licensee Representatives. The following issues/ concerns associated with the operation of the facility were brought forward and discussed with the Licensee/ Licensee Representative(s): Complaint history/ pending complaints and compliance of the facility with all applicable laws and regulations. The involvement of Cassondra Bradford, Administrator of PACIFICA SENIOR LIVING BAKERSFIELD 157204130 and PACIFICA SENIOR LIVING BAKERSFIELD MEMORY CARE 157204131 in operations of any other licensed facilities including but not limited to PACIFICA SENIOR LIVING MERCED 247206921. It was determined during this meeting that Cassondra Bradford, Administrator of PACIFICA SENIOR LIVING BAKERSFIELD 157204130 and PACIFICA SENIOR LIVING BAKERSFIELD MEMORY CARE 157204131 acts as an owner's representative in terms of conducting oversight for a lease (asset management) for community in the city of Merced. Note: Pacifica owners collectively known as PACIFICA BAKERSFIELD LP and/or PACIFICA SENIOR LVG MGT It was determined during this meeting that Cassondra Bradford, Administrator of PACIFICA SENIOR LIVING BAKERSFIELD 157204130 and PACIFICA SENIOR LIVING BAKERSFIELD MEMORY CARE 157204131 acts as a Regional Director for a total of five (5) other facilities under the umbrella of PACIFICA BAKERSFIELD LP and/or PACIFICA SENIOR LVG MGT .
