On 10/09/25, Licensing Program Analyst (LPA) M. Yang arrived unannounced to conduct initial complaint investigation. LPA introduced self, stated the purpose of the visit and met with Resident Service Director Casey Gonzales who stated Administrator Douglas Rice is unavailable. The purpose of the visit is to conduct a health and safety visit on the resident’s in care. It was informed that R1 was sent out to the hospital with a swollen eye. Interviews were conducted, records were received, and the facility was toured. The information provide will be reviewed; a follow up case management will be conducted if necessary. An exit interview was conducted. A copy of this report was provided to Resident Service Director, whose signature confirms receipt of this report.
