On 10/03/24, Licensing Program Analyst (LPA) M. Yang arrived unannounced to deliver findings on the above allegations. LPA introduce self, stated the purpose of the visit and met with Assistant Administrator Sharnpreet "Pree" Malhi. During the course of the investigation, interviews were conducted, and records were reviewed. The resident has not have any visitors that were denied visitation. The resident is not able to leave the facility unassisted without consent from conservator and have left the facility on multiple occasions with conservator. Based on interviews conducted and records reviewed, the preponderance of evidence standard has not been met, therefore the above allegations are found to be UNSUBSTANTIATED. Exit interview conducted. A copy of this report was provided to Assistant Administrator.
