What the inspector wrote

On 04/13/26, Licensing Program Analyst (LPA) M. Yang arrived at the facility for an unannounced case management visit to conduct a health and safety check on the residents in care. LPA met with staff Rajwinder Kaur, discussed the purpose of the visit and was permitted entry into the facility. Licensee/Administrator Tina Malhi and Assistant Administrator Sharnpreet "Pree" Malhi was called and arrived shortly. This case management visit is being conducted for an incident report received by the Department on 04/08/26 for the unexplained death of R1 on 04/04/26. LPA toured the facility with Licensee. The residents were observed in the common area and sleeping in their bedrooms. Facility appeared to be clean with no obstruction. Facility was observed to have operating and functioning electricity during inspection. The food pantry was observed locked. Food supply was checked and appeared to have an adequate supply in facility pantry, refrigerator, and freezer. LPA received copies of R1’s file. The information provided will be reviewed; a follow up case management will be conducted if necessary. A deficiency is being cited on the attached Lic 809D in accordance to California Code of Regulations, Title 22, Division 6. Exit interview completed. A copy of this report and appeal rights was provided to Licensee, whose signature on this form confirms receipt of this report.
