Facility must keep complete hospice care plan on file

Based on record review, the licensee did not comply with the section cited above when LPA reviewed R1’s file who is currently receiving hospice care with no current hospice care plan on file, which poses a potential health or personalrights risk to persons in care.

Based on observation, records reviewed, and interview conducted, R1 is not on hospice and using full rail hospital bed with no doctor’s orders, which poses/posed a potential health and safety and personal rights risk to the resident in care.

Record dose details and resident medication response

Based on observations, records reviewed, and interviews conduct, R1’s PRN medication was order on 11/07/23 and recorded centrally stored record and did not record in the resident’s MARs, which poses/ posed a potential health and safety risk for the person in care.

Requirements for locking exterior exits

Based on observation, the licensee did not comply with the section cited above when knives and medication cups fill with medications were stored in malfunction kitchen drawers this poses an immediately health, safety or personal rights risk to persons in care.

Document each PRN medication dose taken

Based on observation, the licensee did not comply with the section cited above when LPA and Licensee reviewed all the residents’ MARs and observed all the residents MARs not completed correctly. The staff stated medications for 4/30/23 AM, 04/30/23 PM, and 05/01/23 AM was administered; however the staff did not sign nor initial medications was given for the residents in care this poses a potential health, safety or personal rights risk to persons in care.

Based on observation, the licensee did not comply with the section cited above when gnats were observed to be in the food pantry due to the foul odor of onions that had sprouted,which poses a potential health, safety or personal rights risk to persons in care.
