Inspector’s narrative

What the inspector wrote

On 08/06/2025, Licensing Program Analyst (LPA) M. Yang arrived unannounced to conduct an annual inspection. LPA introduced self, stated the purpose of the visit and requested to meet with the Administrator. LPA met with caregiver Rechilda Castaneda. LPA toured facility with caregiver. Licensee Michelle Ligon and Administrator/Licensee, Jocelyn Ligon was called and arrived shortly. LPA tour facility with Licensee and Administrator. LPA conducted a tour inside and outside of the facility. Residents were observed in the common area and in bedrooms. Facility observed to be clean, odor free and at a comfortable temperature. Common areas were furnished well with adequate seating and lighting available. Last fire drill was completed on 06/01/25. Fire extinguisher serviced on 07/24/25. Kitchen toured, appeared clean, observed a 7-day supply of non-perishable food and 2-day supply of perishable food. Resident rooms appeared clean and had required furnishings. LPA observed an adequate supply of linen. Resident bathrooms were properly equipped with securely fastened grab bars in toilet and tub/shower areas, non-skid mats were observed. Hot water measured at 111.7 and 112.3 degrees F in bathroom 1, 106.9 degrees F in bathroom 2, and 106.4 and 107.1 degrees F in master bathroom. Chemical bottles were observed unlocked in bathrooms and laundry room. Medication observed locked in hall medication cabinet. Exterior tour conducted, side exit open and free of obstructions. Side gate was observed to be self-closing. Smoke detectors and carbon monoxide detectors observed operational during today’s inspection. LPA reviewed a sample of staff’ and residents’ records. A deficiency is being cited on the attached Lic 809D in accordance to California Code of Regulations, Title 22, Division 6. Exit interview conducted. LPA is requesting the following documents be submitted to the Fresno CCL office by 08/12/25: Current copy of Administrator Certificate, Lic 308, current Liability Insurance, Lic 610E, and Lic 500. A copy of this report and appeal rights was discussed and provided to Licensee, whose signature on this form confirms receipt of this document.

    Based on records reviewed and interview conducted, S1 and S2 do not have First Aid, this poses an immediate health and safety risk for the residents in care.

    Based on record review, the licensee did not comply with the section cited above when LPA reviewed staff files and observed no health screening were on file for S1 and S2, which poses a potential health or personal rights risk to persons in care.

    Based on observation and interviews conducted, the licensee did not comply with the section cited above when video audio camera was observed in the common area to video record residents, which poses an immediate health, safety or personal rights risk to persons in care.
