What the inspector wrote

An unannounced Complaint visit was conducted on the date & during the times identified above by Licensing Program Analyst (LPA) K. McClurg & Licensing Program Manager (LPM) See Moua. LPA & LPM met with Administrator (Admin) Ramona Eleco & stated purpose of visit. Previous visit, LPA observed call assistance button to be availble in room for Resident 1 (R1). Button tested & observed to be operational. The Department has investigated the above allegation & determined it to be Unfounded.

Type A. Serious citation. Imminent or substantial risk to children. The regulator requires corrective action immediately and may impose a civil penalty.

Type B. Lower-severity citation. Corrective action required, no imminent risk. The regulator monitors compliance on the next visit.

  • 87303(a)Type B

    Maintain facility in clean, safe, sanitary condition

    Maintenance and Operation-The facility shall be clean, safe, sanitary and in good repair at all times. Maintenance shall include provision of maintenance services and procedures for the safety and well-being of residents, employees and visitors. Cockroaches present in facility in resident rooms. Current Pest Control contract is insufficient to meet the facility need to control cockroaches.

  • Limit staff assistance with self-administered drugs

    Incidental Medical and Dental Care-The licensee shall assist residents with self-administered medications as needed. MARS observed not maintained ensuring that residents are receiving assistance with rx as needed.

  • 87645(h)(2)Type A

    Incidental Medical and Dental Care-Centrally stored medicines shall be kept in a safe and locked place that is not accessible to persons other than employees responsible for the supervision of the centrally stored medication. Variety of unidentified medications & vitamins observed in R1's room - not centrally stored or maintained in a locked location
