Correction check·October 25, 2022

On 10/25/22, Licensing Program Analyst (LPA) L. Salazar arrived at the facility unannounced to conduct Plan of Correction visit. LPA was greeted by Administrator, explained the purpose of the visit and was allowed entry into the facility. During a visit on 09/21/22, deficiencies were cited. Administrator completed the Plans of corrections prior to due date and all deficiencies have been cleared. LPA provided Administrator with a plan of correction letter evidencing the corrections were made. An exit interview conducted. No deficiencies cited.
