What the inspector wrote

Licensing Program Analyst (LPA) S. Doucette and LPM S. Moua conducted a joint inspection. The above complaint allegations were discussed and reviewed with Administrator Ramona (Mona). The Department conducted interviews and reviewed resident’s files. Emergency services were sought for R1 after a fall. R1 was later discharged from the hospital and back to the facility. During an inspection tour, LPA observed R1 at the facility. MARs were reviewed for R2 and medications were administered as prescribed. The above allegations are Unsubstantiated. Exit interview was conducted.

    87303(a) Maintenance and Operation - The facility shall be clean, safe, sanitary and in good repair at all times. This requirement was not met as evidenced by: Based on observation, LPA and LPM observed the ice and juice machines with mold, which poses a potential health and safety concerns to the residents in care.

  • Provide resident hot water for personal care

    Based on observation, the licensee did not comply with the section cited above in by residents rooms measuring above 125 F and under 85F which poses an immediate health, safety or personal rights risk to persons in care.

    Based on records review and interviews, the licensee did not comply with the section cited above by staff not having any current training, which poses/posed a potential health, safety or personal rights risk to persons in care.

    Based on record review and observation, the licensee did not comply with the section cited above by having residents in care that require a higher level of care, which poses/posed a potential health, safety or personal rights risk to persons in care.

  • Assist residents with self-administered medication

    Based on observation and record review, the licensee did not comply with the section cited above by medicaitons were not administered/logged as being administered for several residents in care, which poses an immediate health, safety or personal rights risk to persons in care.

  • Maintain admission agreement and appraisal

    Based on record review, the licensee did not comply with the section cited above by licensee not having admission agreements signed for residents, which poses/posed a potential health, safety or personal rights risk to persons in care.

  • Dignity in personal relationships

    87468.1(a)(1) Personal Rights of Residents in All Facilities- Residents in all residential care facilities for the elderly shall have all of the following personal rights: To be accorded dignity in their personal relationships with staff, residents, and other persons. This requirement was not met as evidenced by: Based on observation, LPAs observed R1 left outside calling for help, which poses a potential health and safety concern.
