What the inspector wrote

have been left unattended at appointments. Interviews revealed R1 was dropped off at an appointment on 10/10/2025 at 9:30 AM and was left in the lobby unattended until staff arrived to pick up R1 at 11:00 AM. Based on interviews, the preponderance of evidence standard has been met; therefore, the above allegation is found to be SUBSTANTIATED. Per California Code of Regulations, Title 22, Division 6, Chapter 8, a deficiency is being cited on the attached 9099-D. A copy of this report with plans of corrections and appeal rights were provided.
