On 6/28/25, Licensing Program Analyst (LPA) M. Medina conducted an unannounced Annual Required Inspection. LPA arrived, introduced self, stated purpose of visit and allowed entrance by House Manager, Kelly Wejmar. Sha-Ron Thomas, Co-Administrator contacted by telephone and arrived a short time later to conduct facility with LPA. . Four (4) residents were present during today's inspection. All were observed to be relaxing at start of facility inspection. Facility tour conducted with Co-Administrator Sha-ron Thomas. LPA observed to be clean, and a comfortable temperature. All residents bedrooms toured, bedrooms observed to be well lit and have all required furnishings. Resident bathrooms toured, LPA observed both toilet and shower areas to have grab bars, shower is also equipped with a skid-resistant surface. Water temperature measured at 108 degrees F at time of inspection. All common areas of facility have adequate seating available for residents in care. Kitchen toured, facility observed to have a 2-day supply of perishable and a 7-day supply of non-perishable food available. Knives observed to be locked and secured in the medication closet in the hallway. Medications observed to be secured and locked in medication closet, medications reviewed were observed to have original labels and to be administered as prescribed. All chemicals and cleaning supplies observed to be secured laundry room which is locked. Smoke detectors and carbon monoxide detectors present and observed operational. Fire extinguisher present with a service date of 8/14/2024. Las fire drill conducted 6/07/25 according to facility records, Outside of facility toured. All exits open free of obstruction. Shaded area for seating available for residents. No hazards observed. Staff and resident files reviewed. P & I reviewed. LPA requested the following documents to be submitted to Fresno Regional Office no later than 7/11/25: LIC 500 (Personnel Roster), LIC 9020 (Register of Facility Clients/Residents), Copy of Liability Insurance. No deficiencies cited during inspection Exit interview conducted. A copy of report will be emailed to Administrator.
