What the inspector wrote

Licensing Program Analysts (LPA) B. Miranda conducted an unannounced visit today February 24, 2026 for the facility’s annual inspection. LPA introduced themselves and was allowed entrance into the facility. LPA met with Administrator Jean Asignacion. Facility is licensed for 6 residents and has a current census of 3. There are 0 residents on hospice and 0 with a health care plan. Water temperature was checked in the kitchen which read at 113.1 degrees Fahrenheit and the common bathroom which read at 114.9 degrees Fahrenheit. Fire Extinguisher was purchased 4/1/2025 and is within the safety regulation period. Carbon monoxide detectors were tested and in working order. Jean Asignacion Administrator's Certification expires 11/12/2027. Staff files were reviewed, and training is not current. Resident files were reviewed, and are not current, physician report is not current for R1 & R2. Facility did not provide a current log for disaster drills that are conducted quarterly. First aid kit on site and complete. Tools and knives are not locked and accessible to residents when in the kitchen. There is a locked storage for medications. LPA inspected the interior and the exterior of the facility including the common living spaces, resident bedrooms and bathrooms, medication storage, kitchen, garage and outdoor areas. Bedrooms were clean, properly furnished, with adequate lighting, and in good repair. Food supply is adequate for 2-day perishable and 7-day nonperishable. LPA observed the following deficiencies and cited during today's inspection per California Code of Regulations, Title 22. On duty caregivers do not have current first aid training or current training. 2 of 3 resident’s physician reports are not current Kitchen appliances have grease/food buildup on outside and inside. Dishwasher has mildew buildup inside. Microwave has food debris on the inside Refrigerator and cabinets have debris Tools in unlocked cabinet & lighters in unlocked kitchen drawer Leak under kitchen sink. Tools in unlocked cabinet in the kitchen Cleaning products accessible in staff bathroom (bathroom was unlocked) Administrator was not able to provide quarterly drill verification. R3 Centrally Stored Medication Log incomplete Raloxifene HCL 60 MG TAB not logged Med count off Raloxifene HCL 60 MG TAB started 11/29/2025- 2/24/2026 (87days), 90 count, 90-87= 3 pills should be left, 5 pills were left in the bottle. No PRN form for R3 Lorazepam Additional lock to front exterior door. LPA's requested the following documents: LIC 500 Personnel Report, LIC 308 Designation of Administrative Responsibility, LIC 610-E the Emergency Disaster Plan and copy of current Administrator’s Certificate to update the facility file. Listed documents shall be sent to Licensing. Exit interview conducted and a copy of this report LIC809, LIC809D, and appeal rights were provided to Administrator Jean Asignacion.

LPA observed dish washer to have debris build up, kitchen appliances having grease/food build up on the outside & inside, and microwave to have buildup on the inside. Kitchen sink has a leak that needs to be fixed. LPA observed refrigerator and kitchen cabinets to be unclean.

LPA observed tools in unlocked kitchen cabinet, knives in cabinet not properly locked, lighters in unlocked kitchen drawer.

    Based on observation, interview, & record review, the licensee did not comply with the section cited above which poses an immediate health, safety or personal rights risk to persons in care.
