What the inspector wrote

On 8/26/2022, Licensing Program Analysts (LPA) M. Medina arrived to conduct a Case Management visit related to concerns reported to Department. The concerns were reported to Department during an unrelated Complaint Investigation. LPA introduced self and stated purpose of visit. LPA met with Francine Reyes, Director of resident services. Facility toured. During the course of complaint investigation conducted it was observed that the air conditioning in room #10 was not working properly and the toilet seat in resident bathroom #1 was missing. Deficiencies cited on the attached 809-D. Exit interview conducted. A copy of this report was provided for facility records. .

    Facility personnel shall at all times be sufficient in numbers, and competent to provide theservices necessary to meet resident needsThis requirement was not met as evidenced by staff not supervising residents in the television room,resulting in R1 assaulting R2, requiring R2 to be hospitalized for fractured ribs and pneumothoraxrequiring chest tube placement. This is an immediate risk to resident health and safety.
