On 08/17/23, Licensing Program Analyst (LPA) M. Yang arrived unannounced to deliver complaint findings on the above allegations. LPA introduced self, stated the purpose of the visit, and met with Director of Residential Services Shellie Whitlock and Administrator Paul Anderson. During the course of the investigation record were reviewed and interviews were conducted. S1 had hit R1 and R2’s arm to get the residents’ attention. S1 spoke inappropriately to R1 and R3 when assisting the residents. S1 had pulled R2’s hair when R2 was trying to get out of the commonly used bathroom. Based on record reviewed and interviews conducted, the preponderance of evidence standard has been met,therefore, the above allegations are found to be SUBSTANTIATED. Under California Code of Regulations, Title 22, are being cited on the attached LIC 9099D. An exit interview was conducted, and a copy of this report and appeal rights was provided to Administrator, whose signature confirm receipt of this report.
