What the inspector wrote

Licensing Program Analyst (LPA) Shawna Doucette and Rachel Bruce conducted an unnannounced visit to investigate a complaint and deliver findings. LPA met with House Manager, and discussed the purpose of the visit. House Manager Diana Diaz responded to the facility to assist with the visit. LPA conducted interviews. Based on interviews R1 was not evicted. R1 required a higher level of care. Although the allegations may have happened or is valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur, therefore the allegations are UNSUBSTANTIATED. An exit interview was conducted and a copy of this report will be provided via e-mail.
