What the inspector wrote

Licensing Program Analyst (LPA) K.Kaur arrived unannounced for a case management visit regarding the incident report (elopement) that was received. LPA met with Licensee Representative Anthony Barbato. LPA interviewed staff about the incident report. Client files reviewed, per client files, client cannot leave the facility unassisted. See attached LIC 809D for deficiency cited in accordance with California Code of Regulations Title 22. Immediate $500.00 civil penalty was assessed. Exit interview conducted with Administrator. Appeal rights and a copy of the reports provided via email.
