What the inspector wrote

Licensing Program Analyst (LPA) Sarah Hurt conducted an unannounced facility visit to investigate the allegation above. LPA met with facility Facility staff, Nancy Cudal, and explained the purpose of today's visit. Regarding the allegation staff administered a medication to a resident without authorization. LPA conducted an interview with the resident, reviewed medication documentation, interviewed facility staff, and medical office staff. The resident is responsible for his own care and admission decisions and is not conserved. Record review showed documentation indicating that the resident declined the medication in September and October 2025. During the LPA’s interview, the resident—who is nonverbal but communicates effectively confirmed that they did not receive the injection and that they continue to refuse it. Staff interviews and documentation confirmed that the resident’s decision was respected and that no injection was administered without consent. This agency has investigated the complaint alleging staff administered a medication to resident without authorization. We have found that the complaint was unfounded, meaning that the allegation was false, could not have happened and/or is without a reasonable basis. No deficincies cited Per title 22 Regulations. Ext interview conducted with facility staff Nancy Cudal, and a copy of this report provided.
