What the inspector wrote

On 11/21/2025, Licensing Program Analyst (LPA) J. Duarte and B. Miranda, arrived unannounced to conduct required Annual inspection. LPAs introduced self, stated the purpose of the visit, and was greeted by staff Margarita Pena. LPA J. Duarte contacted Licensee, Jason Johnson via telephone and advised of today's visit. Administrator Diana Diaz arrived shortly after and a tour was conducted. The facility was observed to be at a temperature of 75 degrees F, clean and in good repair. The living room has sufficient seating for residents. The kitchen was toured and LPA observed a two-day supply of perishable and seven-day non-perishable food. The facility stores kitchen knives in a locked hallway closet. A fire extinguisher was observed in the kitchen, with a service date of 05/23/25. All bedrooms were toured and observed to have the required furniture and adequate lighting. Bathrooms were observed operational, with non-skid strips and grabbed bars observed for the showers and toilets. The hallway restroom hot water measured at 109 degrees F and the hot water to the restroom connected to the room measured at 111 degrees F. Chemicals were observed stored and locked in the garage. The facility has a dryer and washer in the garage. The facility has an additional freezer in the garage with an additional food supply. Outside of the facility toured and observed to be free of debris. The carbon monoxide/smoke detector was observed operational during inspection. Medications observed kept locked in facility office. A first aid kit is also stored in a hallway closet. The last fire drill was conducted on 11/6/2025, per staff records. A sample of resident files and staff files were also reviewed and observed to have required documentation. Continued on LIC 809C. Continued from LIC 809. The following deficiencies were observed during visit: LPAs observed the exit to bedroom #3 on the facility sketch to be obstructed with R1's bed. LPAs observed medication is not being properly logged with the centrally stored information. LPAs and administrator observed R3 to have an expired restricted home health care plan. The facility utilizes oxygen for a resident in care and LPAs and administrator did not observe any signs were posted to indicate oxygen is in use. The emergency disaster plan does not have detailed and complete responses. An office meeting will be scheduled with the licensee to discuss the deficiencies observed on today's inspection and citations will be issued if warranted. An exit Interview was conducted. The Administrator had to leave; however, licensee authorized staff Aaliyah Meza, to sign this report. A copy of this report was provided to staff Aaliyah.
