What the inspector wrote

On 08/28/25, Licensing Program Analyst (LPA) J. Duarte arrived unannounced to conduct the required Annual inspection. LPA introduced self, stated the purpose of the visit, and met with staff Shirley Ralda.. Staff contacted Administrator (AD) Diana Diaz, and AD arrived shortly later. LPA toured the facility with AD. The facility was observed to be at a temperature of 76 degrees F, clean, in good repair, and no passageway obstructions were observed. The living room has sufficient seating for residents. The kitchen was toured and LPA observed an adequate supply of perishable and non-perishable food. The facility stores kitchen knives in a locked hallway closet. A fire extinguisher was observed in the kitchen, with a service date of 05/23/25. The kitchen hot water measured at 96.5 degrees F. The dining room has a fireplace with a cover. Bedrooms were toured and observed to have the required furniture and adequate lighting. Bathrooms were observed operational, with non-skid strips and grabbed bars were observed. The hallway restroom hot water measured at 98.8 degrees F and the hot water to the restroom connected to the room measured at 95.7 degrees F. Extra linens and towels observed stored in a hall closet. Chemicals were observed stored and locked in the cabinet under the kitchen sink. The facility has a dryer and washer in the hallway. The back patio was toured and observed to be free of debris with adequate seating available for residents. The back back patio has a self latching gate. The carbon monoxide/smoke detector was observed operational during inspection. Medications observed kept locked in a hallway closet. A first aid kit is also stored in the medication closet. All resident files and a sample staff files were reviewed and observed to have required documentation. Continued in LIC 809C. Continued from LIC 809. The facility installed a sliding glass door in one of the bedrooms and did not notify or submit proof to Community Care Licensing (CCL) of the alteration. Facility is to provide documentation and an updated sketch to CCL. In addition, faucets used by residents for personal care, shall deliver hot water of not less than 105 degrees F and not more than 120 degrees F. This requirement was not met as faucets delivered hot water at less than 105 degrees F. The hot water temperature was corrected to be within regulation while LPA was at the facility. Deficiencies are being cited on the attached LIC 809D pages, in accordance to California Code of Regulations, Title 22,Division 6. An exit Interview was conducted. A copy of this report and appeal rights were emailed to AD. In addition, please provide an updated sketch to reflect the alterations that were made to the facility by POC due date. Please submit the above forms/information to Fresno CCL by: 09/04/2025.

Licensing was not advised of the proposed change and/or an updated facility sketch to reflect alterations was not provided to licensing, which poses/posed a potential health, safety or personal rights risk to persons in care.
