What the inspector wrote

On 09/12/25, Licensing Program Analyst (LPA) J. Duarte arrived unannounced to conduct the required Annual inspection. LPA introduced self, stated the purpose of the visit, and met with staff Shirley Ralda. LPA contacted licensee Jason and he stated that he will send someone to conduct a tour with LPA. Administrator (AD) Diana Diaz arrived shortly later. LPA toured the facility with AD. The facility was observed to be at a temperature of 73 degrees F, clean, in good repair, and no passageway obstructions were observed. The living room had sufficient seating for residents. The kitchen was toured and LPA observed an adequate supply of perishable and non-perishable food. The facility stores kitchen knives in a locked kitchen cabinet. Chemicals were observed stored and locked in the cabinet under the kitchen sink. A fire extinguisher was observed in the kitchen, with a service date of 05/23/25. The dining room has a fireplace with a cover. Bedrooms were toured and observed to have the required furniture and adequate lighting. Bathrooms were observed operational, with non-skid strips and grabbed bars were observed. The hallway restroom hot water measured at 113.4 degrees F and the hot water to the restroom connected to the room measured at 110.9 degrees F. Extra linens and towels observed stored in a hall closet. The facility has a dryer and washer in the hallway. The back patio was toured and observed to be free of debris with adequate seating available for residents. The back back patio has a self latching gate. The carbon monoxide/smoke detectors was observed operational during inspection. Medications observed kept locked in a hallway closet. A first aid kit is also stored in the medication closet. All resident files and a sample staff files were reviewed and observed to have required documentation. Continued in LIC 809C. Continued from LIC 809. The facility installed a sliding glass door in one of the bedrooms and did not notify or submit proof to Community Care Licensing (CCL) of the alteration. LPA observed R1's bed rail to be extended to more than half the length of the bed. Licensee did not ensure a restricted health care plan for R3's restricted health condition. Deficiencies are being cited on the attached LIC 809D pages, in accordance to California Code of Regulations, Title 22,Division 6. An exit Interview was conducted. A copy of this report and appeal rights were emailed to AD.     Based on observation and interview, the licensee did not comply with the section cited above in that R1's bed rail extends to more then half the length of the bed. Administrator provided a doctor note for R inidicating " Ok to use hospital bed with 1/2 rail." However, the rails extends more than half of the length of R1's bed, which poses/posed a potential health, safety or personal rights risk to persons in care.
