What the inspector wrote

On 08/23/23, Licensing Program Analyst (LPA) M. Yang arrived unannounced to conduct an Required Annual inspection. LPA introduce self, stated the purpose of the visit and met with Administrator Laura Dobson. All two resident was present during inspection. The facility has 6 single occupancy rooms located in the 300 wing of the Hoffmann Hospice Home. The facility was observed to be at a comfortable temperature, clean, in good repair, and no passageway obstructions or fire hazards were observed inside or outside. Medications were observed in medication cart in locked medication room in the facility. LPA review medications and MARs. Facility kitchen was toured. Food supply was checked and appeared to have an adequate supply in facility pantry, walk in refrigerator, and walk in freezer. Temperature maintained for refrigerator at 37 degrees F and freezer temperature log was documented at -2 degrees F. Fire extinguisher was observed with a service date of: 09/15/22. Residents’ bedrooms were observed to have required furnishings and with adequate lightening. Bathrooms were properly equipped and operational. Securely fastened grab bars and non-skid flooring was observed in bathrooms. Hot water temperature was tested at 105.4 degrees in resident 1 bathroom and 115.8 degrees F in resident 2 bathroom. Carbon monoxide and smoke detectors was observed. Carbon monoxide was tested and operational. Outside of facility toured and observed to be free of debris. Adequate outside seatings observed available for residents. Chemicals are stored in locked housekeeping closet, inaccessible to residents. All residents’ file reviewed to have update emergency contacts, Admission agreement, and Pre-Appraisal. Staffs’ files were also reviewed. Staff files were observed to have current First Aid/CPR. No deficiencies issued during this inspection. Exit Interview conducted. The following documents are requested and submitted to Fresno CCL by: 08/29/23. Forms requested: Lic 308, Lic 400, Lic 500, Lic 610E, current liability insurance and current Administrator certificate. A copy of this report was provided to Administrator, whose signature on this form confirms receipt of this report.
