Inspector’s narrative

What the inspector wrote

On 03/24/26, Licensing Program Analyst (LPA) M. Yang arrived unannounced to conduct an Annual Inspection. LPA introduced self, stated the purpose of the visit, and met with Licensee/ Administrator (L1) Yaninee Asawadilokchai/. LPA toured facility with L1. All one resident was present during the inspection. The facility was observed to be at a comfortable temperature, clean, in good repair, and no passageway obstructions or fire hazards were observed inside. Knives observed unlock under kitchen counter. Chemicals observed unlocked under kitchen sink and in garage cabinet. Fire extinguisher was observed with purchased date 10/02/25. Last fire drill completed 03/01/26. LPA observed 2-day perishable food was observed and 7 day non-perishable food was observed. Freezer temperature was at 0 degree F and refrigerator temperature maintained at 37 degrees F. Extra linens and towels were observed in hall closet. Resident’s bedrooms were toured and observed to be adequately furnished and with adequate lighting. All bathrooms are observed with securely fastened grab bars and non-skid mats. Hot water temperature was tested range at 109 degrees F in hall bathroom and 114.4 and 114.8 degrees F in vacant master bathroom. Outside of facility toured and observed free of debris. Adequate outdoor seating was observed available for the residents.Resident’s MARS and Centrally Stored Medication Records were reviewed. Medications were checked. All staff and resident files were reviewed. Smoke detector and carbon monoxide were observed operational and functional during visit. A deficiency is being cited, per California Code of Regulations, Title 22, Division 6, see attached Lic 809D. A civil penalty is being assessed see attached Lic 421IM. Exit Interview conducted. The following documents are requested and submitted to Fresno CCL by: 03/30/26. The following updated forms were requested: Lic 308, Lic 500, Lic 9020, current liability insurance, and current administrator certificate, and Lic 610E. A copy of this report and appeal rights was provided to Licensee, whose signature on this form confirms receipt of this report.

    Based on interview conducted, observations, and records reviewed, all of R1’s medication were administered and not record in the resident’s MAR. R1’s medication Incruse Ellipta Inhalation and Polyethylene Glycol were not recorded in the MAR, which poses/posed an immediate health and safety risk for the person in care.

  • Record centrally stored prescriptions and refill data

    Based on interview conducted, observation, and records reviewed, all the resident’s current medications were not recorded in Centrally Stored Medication (Lic 622) record, poses/posed a potential health and safety and personal rights risk to the resident in care.
