On 12/20/2023, Licensing Program Analyst (LPA) Walton arrived unannounced to conduct a case management visit. LPA introduced self, stated the purpose of the visit and requested to meet with the Administrator. LPA met with Administrator, Nicole Morehead. The purpose of today's visit is to amend reports related to complaint number 24-AS-20230613143211. Reports were amended and the deficiency issued for section 87468.1(a)(1) has been amended to a type B deficiency. No deficiencies issued during this inspection. Exit interview conducted. A copy of this report was discussed and provided to Administrator, Nicole Morehead, whose signature on this form confirms receipt of this document.
