On 08/01/25, Licensing Program Analyst (LPA) M. Yang arrived unannounced to conduct initial complaint investigation. LPA introduced self and stated the purpose of the visit. LPA met with Administrator Wendy Lee and delivered complaint findings on the above allegation. During the complaint investigation, interviews were conducted, received copies of records, and the facility was toured. R1 confirmed R1 received MRI from hospital visit. Follow up appointments were scheduled. Based on interviews conducted and records reviewed, R1 received x-rays and MRI for resident’s injury with follow up appointments, therefore the preponderance evidences has not been met, the above allegation is found to be UNSUBSTANTIATED. An exit interview was conducted. A copy of this report was provided to Administrator, whose signature on this form confirms receipt of this report.
