On 02/25/26, Licensing Program Analyst (LPA) M. Yang arrived unannounced to conduct initial complaint investigation. LPA introduced self, stated the purpose of the visit, and met with Administrator Wendy Lee. During the course of the investigation, the Department conducted interviews, records were reviewed and toured the facility. Based on interviews conducted, allegation alleging staff did not ensure resident are spoken to in an appropriate manner and staff worked while under the influence of alcohol impairing their ability to provide adequate care and supervision to the residents, the preponderance of evidence standard has not been met, therefore, the above allegations are found to be UNSUBTANTIATED. Exit interview conducted. A copy of this report was provided to Administrator.
