What the inspector wrote

On 12/10/24, Licensing Program Analyst (LPA) M. Yang arrived unannounced to conduct investigation and deliver complaint finding on the above allegation. LPA introduced self, stated the purpose of the visit and met with House Supervisor Diana Diaz. During the course of the investigation, the Department conducted interviews, records were reviewed, and facility was toured. Based on observation, records reviewed, and interviews conducted with staff and resident, the resident had planned to move out of the facility, did move out of the facility, and no longer reside at the facility. Based on record reviewed and interviews which were conducted, the preponderance of evidence standard has not been met, therefore the above allegation is found to be UNSUBSTANTIATED. Exit interview was conducted. A copy of this report was provided to House Supervisor, whose signature confirms receipt of this report.
