Inspector’s narrative

What the inspector wrote

On 01/14/2026, Licensing Program Analysts (LPAs) J. Duarte and Licensing Program Manager (LPM), A. Walton, arrived at the facility unannounced to conduct an annual inspection. LPA and LPM were granted entry by staff and Administrator Diana Diaz arrived shortly after. A facility tour was conducted with Administrator Diana Diaz. Facility observed to be well lit, odor free, and comfortable temperature. The facility living room and dining room were observed to have adequate seating available. The dining room has a fireplace with a cover. Resident bedrooms toured, bedrooms observed to have all required furnishings. The water temperature measured at 105 degrees F in both restrooms. The restrooms were observed to have grab bars in the shower and near toilet, and non-skid mats available. LPA observed kitchen to have all sharps and utensils locked and secured in kitchen drawers. The facility has a locked closet designated to store medications, hygiene products, and a first aid kit. The facility has a laundry room with a washer and dryer. The facility stated that they do their own laundry. Chemicals and detergent were observed locked in a kitchen cabinet. LPA checked food supply. The outside of facility was toured. All exits open free of obstruction. Fire extinguisher present with a service date of 05/23/2025. Smoke and Carbon Monoxide detectors present and observed operational. Centrally stored medication and MARS were reviewed. During medication review it was found that staff are performing glucose testing for resident in care. Continued on LIC 809C. Continued from LIC 809. A deficiency is being cited on the attached 809D in accordance to California Code of Regulations, Title 22, Division 6. An exit Interview was conducted and a copy of this report was provided to Diana Diaz.
