What the inspector wrote

On 11/24/2025, Licensing Program Analysts (LPAs) J. Duarte S. Doucette, arrived at the facility for unannounced visit to conduct an annual inspection. LPA's were granted entry by Staff Karla Chagolla. LPA J. Duarte contacted Licensee Jason to advise of today's visit and Administrators Diana Diaz and Elizabeth Ramos arrived shortly after. A facility tour conducted with Administrator Diana Diaz. Facility observed to be well lit, odor free, and comfortable temperature. The facility has not admitted any residents since being licensed. The facility living room and dining room observed to have adequate seating available. Resident bedrooms toured, bedrooms observed to have all required furnishings. The water temperature measured at 116 degrees F. Bathrooms observed to have grab bars in the shower and near toilet, and non-skid mats available. On the facility sketch, bedroom #2 has a window; however, in place of a window, LPAs observed a sliding a glass door. LPAs observed kitchen to have all sharps and utensils locked and secured in kitchen drawers. The facility has a locked closet designated to store medications, hygiene products, and a first aid kit. A separate locked hall closet was observed utilized to store chemicals. The outside of facility was toured. All exits open free of obstruction. Fire extinguisher present with a service date of 05/23/2025. Smoke and Carbon Monoxide detectors present and observed operational. Continued on LIC 809C. Continued from LIC 809. An office meeting will be scheduled with the licensee to discuss the deficiencies observed on today's inspection and citations will be issued if warranted. An exit Interview was conducted and a copy of this report was provided to Elizabeth Ramos.
