On 09/04/2025, an informal meeting was held at the Fresno Regional Office. The purpose of this meeting was to discuss recently identified issues/concerns associated with the operation of the facility. The informal meeting process was explained during this meeting. The following were in attendance at this meeting: Licensee, Miriam Arias Administrator, Angel Zuzueta Licensing Program Manager I, Alexandria Walton Licensing Program Analyst (LPA), Shawna Doucette This meeting was held to discuss the violations that were observed by the LPA during the annual inspection that was conducted on 08/13/2025. Technical Support Program (TSP) was discussed and offered. TSP offer was accepted; a referral will be sent on behalf of the Licensee. Licensee agrees to submit a plan on how regulations will be met and remain in compliance by 09/12/25. Exit interview was conducted. A copy of this report was discussed and provided to the Licensee, whose signature on this form confirms receipt of this document.
