Post-licensing visit·October 23, 2025

On 10/23/25, Licensing Program Analysts(LPAs) L. Salazar and M. Vega arrived to the facility unannounced to conduct the post licensing visit. LPAs were greeted by staff, stated the purpose of the visit and were allowed entry into the facility. Administrator was called and arrived to the facility shortly after LPAs arrival. LPAs toured the facility inside and out. LPAs observed 1 resident in care at the time of visit. Resident is on hospice, hospice binder was available for review. LPAs reviewed R1's facility file and staff records and observed the required documentation. LPAs observed the required 2 day supply of perishable and 7 day supply of non-perishables. Water temperature was tested and measured at 105 degrees Fahrenheit. . Required postings were observed. Facility temperature read at 77 degrees Fahrenheit. No deficiencies cited on today's visit.
