Pre-licensing visit·March 24, 2026

On 3/24/26 Licensing Program Analyst (LPA) M. Garza arrived at the facility for an announced pre-licensing visit. LPA was met by Licensee, Vicent Dang, explained reason for visit and was permitted entry into the facility. LPA completed a tour of the facility inside and out with Licensee. The facility is a single story 4 bedroom, 3 bath home with a 2 car garage in the front of the home. Fire clearance was granted for 6 non-ambulatory residents only. There are no residents currently living in the facility during the time of visit. Facility's phone number is (661) 412-4366. Upon entry, LPA observed the required postings to be present. Common areas observed to be well lit and have adequate furnishings and lighting. LPA observed the front room to be set up for an office. Each of the other rooms are shared. Mattresses appear to be in good condition. An extra supply of linens were located in the hallway closet. LPA observed locked closet in the office identified as the medication cabinet. Kitchen observed to have dishes, plates, and utensils. Knives locked and secured in the office cabinet drawer. Supply of personal hygiene, grooming products, cleaning supplies and chemicals observed. First aid kit contained all required items. A fire extinguisher was purchased on 12/3/2025 and observed fully charged. Smoke detectors and carbon monoxide detectors present and operational at time of visit. Exits were open and free of obstructions. LPA observed side gate to be unlocked and self-latching. Covered seating area observed outside. No deficiencies cited during todays visit. LPA will notify CAB Comp III was completed with Licensee. Exit interview completed with Licensee. A copy of this report provided.
